How To Request Your Medical Records

Texas Health Center for Diagnostics & Surgery (THCDS) respects the health information rights of its patients (and their applicable guardians/legal representatives, if any), including the right to access their protected health information.

Hospital patients can submit a request for records using one of the two methods below.  

If you received care prior to March 1st, 2021

Please use the link below to learn how to access your medical records.  

If you received care on or after March 1st, 2021

Use Texas Health's MyChart to request medical records. From the menu in MyChart select Request Medical Records.
